I've had a couple of primers go in sideways. I had one primer that was just plain loose. (Can't explain it. New brass. None of the rest were loose.) I loaded one dud. I'm convinced I contaminated the primer with dirty fingers and when I had a nice doe standing about 30 yards from me, click---Twice. (I was using an Ishapore and recocked without excessive noise.) They heard the small clicks and eventually ran off. Next round was fine. I short charged a round once, but figured it out right away and pulled the bullet and started over. Nothing more serious. I got mixed up on which powder I had poured, so I threw it away (it wasn't that much) and started over with what I knew I wanted. My most significant incident wasn't strictly speaking an accident, but a procedural problem. I was doing the initial forming from 35 Whelan brass to 338-06 JDJ. Those JDJ rounds have a very sharp shoulder. I scrunched a bunch of rounds before I figured I had to get a start on the mouth before I tried to jam it up in that sharp cone. That wasn't dangerous, but it was somewhat costly. :o
